Patient- and Family-Centered Care

The staff of YRMC recognizes the very important role that patients and family members can play in ensuring the health and well-being of patients of all ages. This recognition is embodied in the term: Patient- and Family-Centered Care.


Patient- and Family-Centered Care is an approach to the planning, delivery, and evaluation of health care that is grounded in mutually beneficial partnerships among health care providers, patients, and families.


It leads to better health outcomes, wiser allocation of resources, and greater patient and family satisfaction


Patient- and Family-Centered Care Partners (volunteer positions) are one of the more formal roles for previous patients and families to participate in as collaborative partners in quality improvement and in the redesign of health care.


YRMC embraces the four core concepts of Patient- and Family-Centered Care:

  1. Dignity and Respect. At YRMC, health care providers listen to and honor the patient's and family's perspectives and choices. Their knowledge, values, beliefs and cultural background are incorporated into the planning and delivery of care.

  2. Information Sharing. YRMC health care providers communicate and share complete and unbiased information with patients and families in ways that are affirming and useful. Patients and families receive timely, complete and accurate information in order to effectively participate in care and decision making.

  3. Participation. At YRMC, patients and families are encouraged and supported in participating in care and decision making at the level they choose.

  4. Collaboration. Patients, families, health care providers and YRMC leaders collaborate in the development, implementation and evaluation of policies and programs; in the design of health care facilities; in professional education and in the delivery of care.

YRMC is seeking volunteer Patient- and Family-Centered Care Partners to provide valuable input for Patient- and Family-Centered Care on an ongoing basis. Volunteers of all ages are needed to serve in a variety of roles. We invite you to learn more about how you can serve your community by participating in this important program.
