YUMA CRS CLINICS

In this CRS New Member Orientation Manual you will find a list of providers, their specialty and the language(s) they speak. You have the right to request to be seen by a CRS Clinic provider who speaks your language, or request a second opinion. Yuma CRS will attempt to meet the request depending on the availability of the provider and the timeframe that your child needs to seen.

Yuma CRS has designated interpreters as well as volunteers who assist with this process on a regular basis. For those members who are deaf, hearing and/or speech impaired, CRS staff will utilize telecommunication services by dialing 711. Yuma CRS also has an American Sign Language Interpreter available.

crs_3.jpgIn addition, Yuma CRS has access to Cyraphone; this service allows a staff member to use an interpreter via the telephone.

Yuma CRS offers certain specialty clinics to the CRS members. Some of the specialists live in the Yuma area and other specialist come from Tucson and Phoenix.

Sometimes the unexpected occurs, flight times change; mechanical delays and/or failure and occasionally we do get bad weather.

This is why we ask that you bring adequate amount of supplies for your child (i.e., diapers, snacks, formula and other necessary items), and be prepared to have a waiting period. Waiting time may vary with each specialty clinic, so please be patient.

WHAT TO EXPECT WHEN YOU ATTTEND A SPECIALTY CLINIC:

Audiology Clinic:

Audiology clinic is generally scheduled once a month concurrently with an Ear Nose & Throat Clinic (ENT). Some audiology examinations are scheduled at the physician's office based on the member's needs.

CRS members will have their hearing tested and/or checked. Some members will be fitted for hearing aides and ear molds.

  • Hearing aids will be provided to CRS members only.
  • Members may have hearing aides reevaluated annually at the CRS clinic.
  • Hearing aides may be replaced once every three years, unless the member has a change in hearing levels and/or a CRS contracted audiologist require the hearing aides to be replaced.
  • Replacement of lost hearing aids is limited to a one-time replacement per a 12-month period.
  • Hearings aides that are no longer being used by a CRS member are to be returned to CRS.

Please remember to bring hearing aids when attending this clinic.

The audiologists work closely with the ENT specialists and are also part of the Cleft Lip & Cleft Palate Team.

Cardiology Clinic:

Cardiology clinic is scheduled every 6 weeks. The Tucson Cardiology group come from Tucson and consists of an Echo cardio-Technologist, nurse practitioner, and two pediatric cardiologists. The Tucson Cardiology group brings the Echocardiogram equipment.

The Tucson Cardiology Group is responsible for scheduling patients and CRS members in this clinic. The Cardiology Group also sees private pay patients, AHCCCS patients and some adults, as well as the CRS members.

The Cardiac Specialists may request and order certain lab or diagnostic testing to be completed on your child. When a cardiac surgery is required, the surgery will take place in Tucson, at the University Medical Center. The surgery will be scheduled by phone with the Tucson CRS Site and the Tucson Cardiology Group and they will be responsible for notifying you.

Cleft Lip/Palate Clinic:

Cleft Palate clinic is held once a month on a Wednesday evening from 5:30 PM to 8:00 PM. This clinic consists of many specialists, whom we call the “Cleft Palate Team”, and the only time they can all get together is after their private practices are closed. Cleft Palate clinics are postponed during the months of June and July.

The Yuma Cleft Palate Team consist of a: Pediatrician, Plastic Surgeon, Oral Surgeon, Dentist, Orthodontist, Periodontist, Prosthodontist, ENT Specialist, Audiologist, Speech Pathologist, Nutritionist, CRS Case Worker, and the CRS Nurse Coordinator.

The Cleft Palate Team will evaluate your child in an examination room to discuss and agree upon the treatment plan the CRS member is to receive. The outcome of the treatment plan will then be discussed with the family and CRS member.

Most of the follow up work and surgeries will be completed in the offices/private practices of the specialists. The CRS clinic is where the Team meets to discuss and concur on the phases of the treatment plan your child is to receive.

Dental and Orthodontia services will be provided for CRS members with the following conditions only:

  • Cleft lip and/or Cleft Palate
  • Documented significant functional malocclusion
  • Cardiac conditions that are at risk of subacute bacterial endocarditis
  • Cerebral spinal fluid diversion shunt that are at risk for subacute bacterial endcarditis
  • Dental complications arising as a result of treatment for a CRS condition

Endocrinology Clinic:

Endocrinology is the study of the glands and hormones of the body and their related disorders. Endocrinology clinic is held once a month in the afternoon. You can expect the doctor to do a physical examination, discuss your child's current prescriptions and discuss with you the results of current lab or diagnostic tests. The doctor may also order new lab or diagnostic tests to be done or may order new prescriptions.

ENT Clinic:

Ear Nose and Throat (ENT) clinic is scheduled monthly. ENT clinic tends to be scheduled concurrently with an Audiology Clinic. The Ear Nose & Throat specialist examines the member's ear, nose and throat and also, works closely with the audiologists. The ENT specialists are also members of the Cleft Lip & Cleft Palate Team.

Enrollment/General Pediatrics Clinic:

Enrollment/General Pediatrics clinic is held once a week by a pediatrician or the Yuma CRS Medical Director who is also a pediatrician. Your child may be scheduled in an Enrollment/ General Pediatrics Clinic for one or all of the following reasons:

  • To be enrolled into the CRS Program.
  • The member is not assigned to a primary care physician/pediatrician.
  • The member is taking more than one medication.
  • The member is also receiving services at another CRS site.
  • The member is scheduled for a procedure that will require sedation.
  • The member is diagnosed with cleft palate.
  • Applicants being enrolled into the CRS program will be seen and assessed by the CRS Eligibility Worker, CRS Case Worker, CRS Nurse and the pediatrician. You will be informed prior to your appointment so that you may come prepared for a longer stay.

Feeding Clinic:

Feeding clinic is held once a month. In this clinic a Nutritionist, Speech Pathologist and an Occupational Therapist evaluate your child's oral motor skills, chewing, swallowing, movement of the tongue, sensory/texture, self-feeding, calories, and height/weight.

Part of the evaluation is feeding and observing your child eat the foods you are asked to bring from home. So it is important that your child is hungry and ready to be fed when attending this clinic. You will be asked to provide three days feeding record prior to the scheduled appointment.

Gastroenterology Clinic:

Gastroenterology clinic is scheduled once a month. Gastroenterology clinic is a service that is open to the Yuma community. Pediatric gastroenterology is a specialty that is not available in Yuma and therefore the gastroenterology specialists will see AHCCCS patients, private pay patients and CRS members in this clinic.

Genetics/Metabolic Clinic:

Genetics/Metabolic clinic is held every 6 to 8 weeks. It is a 1½-day clinic. The physicians come from Tucson and/or Phoenix. Metabolic clinic is held twice a year in January and in July.

These specialists ask many questions about you and your family's medical history. They also request/order lab test to be done on your child and occasionally they will ask the parents if they too would like to have certain lab work done.

Geneticists' research, document, educate, and maintain what they are discovering everyday in Medical Sciences.

Neurology Clinic:

Neurology clinic is held once a month. The Neurologist comes from Phoenix to see Yuma CRS members.

The specialists may order lab and/or diagnostic test on your child. Follow up visits are very important in this clinic due to the medical conditions this specialist sees. Blood draws and other types of lab work are important due to the medications your child may be taking.

Nutrition Clinic:

Nutrition clinic is held once a month. CRS members are seen in this clinic when a CRS physician recommends your child to be evaluated by the nutritionist. CRS members may be scheduled for any of the following reasons:

  • Your child is over or under weight
  • Your child has a feeding tube
  • Your child has difficulty swallowing

Ophthalmology Clinic:

Ophthalmology clinic is held every month. The Ophthalmologist comes from Tucson to provide service to the CRS members.

The CRS nurse may check the CRS member's eyes by having the CRS member look at eye charts before being seen by the physician.

Please remember to bring your child's eyeglasses when attending this clinic.

If the CRS member requires eyeglasses the doctor will write a vision prescription. CRS will pay for one pair of eyeglasses or one pair of contact lens per calendar year when the CRS Member has a covered condition. Ultra Violet (UV) tinting and any other type of lens enhancement will only be paid for if it is medically necessary and ordered by the ophthalmologist.

The Ophthalmologist may put drops in the member's eyes to dilate the pupil. This takes about 20 minutes, so the CRS nurse will ask the family to return to the CRS lobby area to wait. This will allow the specialist to continue to see another member. Once the eyes are dilated the member will return to the examination room.

If the member's eyes are to be dilated, the physician will ask the member to wear sunglasses when outside or watching television for 4-6 hours and if applicable: to not drive for the next 4 - 6 hours also.

The specialist may use one or all of the following instruments when examining the eyes:

  • A small light to look into the member's eyes
  • A Slit Lamp
  • Ophthalmoscope

If surgery is required the surgery may be scheduled at Yuma Regional Medical Center (YRMC) or University Medical Center (UMC) in Tucson. The specialist and the type of surgery that is needed will determine the location.

Dr. Jill Caporelli is Yuma's CRS contracted optometrist located at Adobe Eye Care Center. Adobe Eye Care Center will adjust the member's eyeglasses and will replace any lost or loose screws to the frames. If you have any questions please feel free to call Adobe Eye Care Center at 329-9685.

Orthopedic Clinic:

Orthopedic clinic is scheduled 1 to 2 times a month. We have two orthopedic specialists from Yuma that are CRS providers and one orthopedic surgeon from Phoenix that flies in every other month. The orthopedic surgeon from Phoenix brings with him an attending resident from the Phoenix Orthopedic School.

The Yuma orthopedic specialists perform the CRS surgeries at YRMC and the Phoenix orthopedic surgeon performs the CRS surgeries at St. Joseph's Hospital.

Orthopedic Clinics are scheduled concurrently with Orthotics Clinics. When it is required or recommended by the orthopedic specialists the CRS member may also be seen in the Orthotics Clinic. The orthopedic specialists work closely with the certified orthotist, physical therapist and occupational therapist. The orthopedic specialists may order x-rays, MRIs and other diagnostic testing when attending this clinic.

Orthotics Clinic:

Orthotics clinic is generally scheduled concurrently with an Orthopedic Clinic. Orthotics clinics may be held 1 to 2 times per month. The days and time vary depending on the specialists.

Durable Medical Equipment (DME) is equipment and/or a device for orthopedic use. A Certified Orthotist and a Physical or Occupational Therapist work together by fitting your child with the appropriate DME that is ordered by a physician. Serial casting may be done at an Orthotics clinic or the Orthotics Office. Members and their families are shown and given instructions on the wear and care of their DME equipment.

The therapist and orthotist request that the CRS member wear the DME (i.e., braces) to the appointment in order make the appropriate adjustments and/or measurements for new fittings of the DME.

When attending this clinic it is very important to bring the DME with the CRS member.

Pulmonary Clinic:

Pulmonary clinic is scheduled every other month. Pulmonary clinic is a service that is open to the Yuma community. Pediatric pulmonary is a specialty that is not available in Yuma and therefore the pulmonary specialists will see AHCCCS patients, private pay patients and CRS members in this clinic.

Occupational, Physical & Speech Therapies:

Therapies are scheduled only when a CRS physician recommends the services be provided to the CRS member with an eligible covered condition. Therapy sessions generally last one hour. The frequency and duration will vary according to the physician's orders.

Wheelchair Clinic:

Wheelchair clinic is held every other month. APRIA is the contractor that provides wheelchairs to Yuma CRS. APRIA's main office is located in Phoenix. Two qualified skilled APRIA staff members drive to Yuma to assemble and modify wheelchairs in the wheelchair clinic. The CRS Physical or CRS Occupational Therapists and the CRS nurse coordinator also participate in the wheelchair clinic.

CRS will provide and modify wheelchairs for CRS members only. Wheelchairs will only be replaced when the CRS members have outgrown their wheelchair. Modification to the wheelchairs will be done only when functional and structural modification is requested by a CRS physician or on an as needed basis.

The first time visit to this clinic will consist of measuring and fitting the CRS member to the appropriate requirements in making the wheelchair to the specific needs of the CRS member. Western Medical will then bring the completed wheelchair to the follow up visit.

CRS Members and member's family will be shown and given instruction in the care and maintenance of the wheelchair. Follow up visits will be done as requested by a CRS physician or on an as needed basis.

Please remember to bring the wheelchair when attending this clinic.

Wheelchairs that are no longer being used by a CRS member are to be returned to CRS.

Psychology Services:

Psychology services are a short-term crisis intervention service that may be provided to CRS members when a CRS specialist refers the member to be seen. This service is limited to only three (3) visits per calendar year.

The psychologist will do an assessment and an evaluation and the CRS member may be referred to other services available within the community. Additional visits will only be covered when approved by the Yuma CRS medical director. CRS does not cover ongoing psychological counseling or services.

Psychiatry Services:

Psychiatry services are provided to CRS members when a CRS psychologist refers the member to be seen by a psychiatrist. This service is limited to one (1) visit per calendar year, and it must be related to the CRS covered condition. Additional visits will only be covered when approved by the Yuma CRS medical director.
