Yuma Regional Medical Center's Diabetes Self Management Education Program is of the highest quality available and as such is recognized by the American Diabetes Association. The diabetes educators are certified by the American Association of Diabetes Educators and are available to work with you and your family to create a diabetes management plan.
The diabetes self management education program begins with an individual one hour appointment with the diabetes educator to put together a management plan. Then the patient goes to four weekly two-hour group classes throughout the next month. The patient with diabetes is invited to bring along a "significant other" to learn about diabetes self management and to help carry out the self management plan. A final group class of one hour is offered three months after completing the series of classes to review progress made and to identify solutions to problems the patient may have.
The diabetes self management education program will cover the following topics:
- How and when to monitor your blood sugar
- Insulin self-management skills
- What you need to know about oral medications
- Medical nutrition therapy (nutrition counseling) including carbohydrate counting and heart healthy eating
- Exercise counseling
- Stress management and diabetes
- Sick day management for diabetes
- Preventing long-term complications
- Weight loss/maintenance
- Foot Care
- Behavior change
