Mission Statement
To provide comprehensive community-based primary care services, to meet the physical, emotional and social needs of children without a medical home. The program is designed to meet the needs of uninsured children and parents who cannot access services due to a variety of socioeconomic barriers.
Four programs serving six school districts with more than 6,000 elementary school students throughout Yuma County provide basic health care. Your support will keep simple medical problems from turning into life-threatening illnesses.
A pediatrician or nurse practitioner operates out of the following schools:
- Rancho Viejo for Crane Elementary School District
- Pecan Grove for Yuma Elementary School District
- Wellton for Wellton and Mohawk Valley school districts
- Rio Colorado for Gadsden School District
Your donation will help provide:
Diagnosis and treatment for common and simple illnesses and minor injuries, including medication if needed.
Referral and follow-up for serious illnesses and emergencies, including laboratory and/or radiology services at YRMC.
On-site care and consultation, as well as referrals and follow-up for chronic diseases and disorders, such as asthma.
Immunizations, nutrition and exercise counseling.
Consultation with parents and school staff on communicable diseases and care management.
