Y.O.U.T.H. INITIATIVE GRANT APPLICATION
(Yumans' Obesity Undermines Their Health)
The YOUTH Initiative of the Foundation of Yuma Regional Medical Center provides a $100,000 annual fund to support local non-profit organizations that pursue goals to reduce obesity in a defined Yuma-area population. Basic guidelines include:
- No award will exceed $ 10,000.
- All awards are for one fiscal year (October 1 through September 30)
- Though funding of all programs and projects and their amounts are limited to one year, each is always competitively renewable.
- Preference will be given to projects/programs directed at youth, but will consider any that seriously demonstrate the high likelihood of inducing long-term healthy lifestyle changes on any target population in Yuma County.
Applications for the 2009-2010 Y.O.U.T.H. Initiative grants must be turned in to the Foundation of YRMC office (address below) by Monday, June 1, 2009 at 5:00 p.m. Each will be reviewed by the YOUTH Initiative Committee. The full application must include:
- A letter of introduction
- Completed application form including required signatures
- A narrative not to exceed one single-spaced page (12 or 14 point font) containing the following:
- A detail showing the demonstrated need for the proposed project/program, including data about obesity and overweight in the target (or similar) population.
- A set of specific objectives that define the expected outcomes to be achieved within the grant year.
- An annual budget and its justification that shows where the money will be spent to meet each project/program objective.
- A copy of the organization's IRS designation letter.
Award recipients must present a year-end report that cites data quantifying the extent to which the objectives of the application were met. It must also include at least one anonymous personal success story that can be shared with the community.
